Patient Responsibilities
Please carefully read the Albrecht Free Clinic Patient Responsibilities. In order to provide a professional environment, patients must adhere to these guidelines. They have been designed to optimize health care outcomes. Failure to comply with the guidelines may result in dismissal from Clinic.
The physician, medical and administrative staff donate their services to the Albrecht Free Clinic, however all patients are encouraged to contribute to the Clinic’s operations by providing a donation at each visit/encounter. Please make your Clinic donation a priority. We ask that you participate in your generous services provided. A donation canister is located at the entrance of the clinic.
Patient Responsibilities - We Expect Patients To:
• Be on time – Please come 15 minutes before scheduled appointments to be processed through intake. If you arrive 15 minutes late you may not be seen.
• Be patient – There may be times when the Clinic is short volunteers and/or the Practitioners run behind schedule. Please arrange your schedules accordingly. This applies to all appointments, medication refills and requests.
• Be courteous & respectful – The physicians, nurses, and clerks are volunteers. They give their time and talent to help. Absolutely no verbal abuse or combative behavior will be tolerated and will result in immediate dismissal.
• Keep all scheduled appointments or call to cancel no less than 24 hours before or be considered a no show. Multiple no-shows will result in dismissal.
• Provide necessary documentation – We may need to ask for additional financial verification and ask that you comply in a timely manner.
• Refill medications on time. Due to limited Clinic hours, medications should be refilled prior to running out.
• You have a partnership with the Albrecht Free Clinic and with your health. All patients shall agree to take all medications prescribed by provider as directed. We advocate that you remain on the treatment plans prescribed to you, and notify us if you have are having any difficulties in maintaining your plan. Not following your medical plan may result in dismissal from the Clinic.
• Keep all scheduled referral appointments with specialists, dentists, lab and x-ray. Your voucher will be voided if you cancel or no-show. AFC has the right to dismiss you from Clinic if you misuse a voucher for any service provided.
We are under no obligation to care for those who do not comply with these principles and/or who are disruptive to our operations. AFC is not federally or state funded, but relies on volunteers, donors and a generous community to care for your medical needs. Every donation will help ensure that the Albrecht Free Clinic is here for your future needs.
